By joining the Diamond Club, I understand and agree to these 5 simple rules:
• 1) I can Cancel Membership anytime after 3-months with 30-Day Notice (No Long-Term Commitment)
• 2) I will 48-hour notice needed to reschedule or cancel to avoid the full fee. • 3) Sessions are not transferable.
• 4) I recognize that due to any unforeseen circumstances, such as a pandemic, fire, or other occurrences, I will receive my therapy sessions via Video Visit rather than in person if I cannot be served in person.
• 4)  Once a year, I may put a "Hold" on my membership by writing to Diamond, if I won't use my membership for a while. There is a $10 monthly fee to hold my account. (This 'hold' service avoids paying the initiation fee again to rejoin and maintains my current rate. When you're ready, simply come back in as a member.) 

I also understand Therapists at Diamond are self-employed and work under their own direction. I assume all liability and responsibility to communicate openly about my health and anything else the therapists should be aware of.
